DOH-Lee, county offer vaccination reminders and tips


The Florida Department of Health-Lee and Lee County Government are offering the following reminders and tips for those with vaccination appointments coming to the county site off Daniels Parkway east of Interstate 75:

– Only those who have appointments can be vaccinated. Visit https://myvaccine.fl.gov/ to pre-register.

– If you need to cancel your appointment, email Lee-PIO@Flhealth.gov to let DOH-Lee know. Please refrain from calling the state vendor phone line to cancel. DOH-Lee will ensure your appointment at the site will be given to another eligible individual.

– Before you arrive for your appointment, visit www.leegov.com/vaccine to print and complete the COVID-19 Vaccine Screening and Consent Form. Even if your appointment is for a second dose, you must complete the form. The form covers any manufacturer’s vaccine.

– You also need to bring government-issued identification, such as a valid Florida driver’s license or other proof of residency. For a list of accepted proof of residency, visit www.leegov.com/covid-19/vaccine/vaccine-faq#residency. Bring water, snacks and any medication you may need during your time at the site.

– When you come to your appointment, do not arrive earlier than 15 minutes before your time. The site typically runs smoothly, so arriving any earlier is not necessary. To see a map for the site’s exact location off Chamberlin Drive, visit the map at www.leegov.com/vaccine.

– The site is outdoors, so come prepared for the weather. Although shade tents and cooling mechanisms are in place, springtime temperatures are increasing and occasional fronts bring rain showers. Wear comfortable clothing and shoes appropriate for the weather conditions.

– This is not a drive-thru site. You will park your vehicle and approach the tents. You can walk or take a tram. The site is ADA-accessible. If you use a mobility aid, be sure to bring it.

The site is not designed to handle “extremely vulnerable” individuals recently added to the governor’s executive order. Physicians, advanced practice registered nurses and pharmacists may only vaccinate those deemed extremely vulnerable to COVID-19 by a physician. DOH-Lee does not use any of these clinicians to vaccinate. Individuals who qualify for vaccination should seek vaccinations at private-sector locations; do not come to the DOH-Lee site.
